Diploma Concern

Use This Form If:

You’ve already received your diploma and have a question or concern about it. For instance, if your diploma was received damaged in the mail, or if you have questions about the information that’s listed on the diploma itself.

There was a hold on your student account that prevented us from mailing your diploma. You can use the Diploma Request Form to request your diploma once that hold has been released.
You graduated and never received your diploma. You can use the Diploma Request Form to let us know so that we can investigate and respond to you with options.
If you are needing a duplicate of a diploma you have already received. Instead, you will need to follow the instructions on our Diplomas and Certificate webpage under ‘Duplicate Diploma/Certificate'.
